“During the pandemic Pharmacists became a safe and trusted port in the storm for many people and that continues in 2023.”

Pharmacists’ roles are vital in supporting patient health outcomes in the community improve patients’ access to care and ensure the quality use of medicine.

In 2019, ‘Quality Use of Medicine’ and ‘Medicines Safety’ was declared Australia’s 10th National Health Priority. The policy advocates a collaborative approach between healthcare professionals, the patient, their carer and/or family. The statistics show:

• Between 10% and 67% of medication histories have at least one error, and up to 33% of these errors have the potential to cause patient harm*

• More than 50% of medication errors occur at transitions of care*

Every point of contact in the health system, especially pharmacist intervention and medication management services, is an opportunity to prevent medication misadventure.
